- If you have any symptoms of COVID-19, as outlined in the pre-screening checklist, we ask that you seek testing and delay immunization until all symptoms have resolved
- If you have travelled in the past 14 days we ask that you defer immunizations until after you have sufficiently self-isolated
- If you have had close contact with COVID-19 defer immunizations until you have sufficiently self-isolated
- Please bring a filled out immunization form (SECTIONS 1 & 2 ONLY) and Covid-19 pre-screening checklist to your appointment or drop in clinic. These can be printed from our forms section. They will also be available for pickup outside the drug store. PENS AND FORMS WILL NOT BE AVAILABLE AT THE CLINIC. YOU MUST BRING YOUR COMPLETED FORMS WITH YOU
- Please wear a mask, these will NOT be provided for you. Let us know upon arrive if you are unable to wear a mask and we will accommodate you
- Sanitize hands upon entering and exiting the clinic
- Families/household try to come together
- There will be one way traffic through the clinic. Please enter through back door (near the post office parking lot), and exit through the front. If there is a lineup please distance yourself from the group ahead of you by the recommended 6 feet.
- The recommended time you are asked to stay in the clinic for observation after immunization is 15 minutes. There will be chairs placed throughout the clinic that you may wait in. Chairs placed less than 6 feet apart are only to be occupied by members of the same household.
- A shorter observation period of 5 minutes will be considered if the vaccine recipient meets the following conditions:
- Patient has had the influenza vaccine in the past and has no known history of allergic reactions to any component of the vaccine
- No history of other post-vaccination reactions to any vaccine (ex. Fainting)
- The vaccine recipient is accompanied by a responsible adult (or parent/guardian in case of children) who will act as a chaperone to monitor the vaccine recipient for a minimum of 15 minutes post-vaccination. Two adult vaccine recipients can monitor each other if both agree.
- Vaccine recipients must not operate a motorized vehicle or machinery for 15 minutes post-vaccination.
- The adult chaperone must be able to call 911 and seek assistance and medical services if required.
- The vaccine recipient and chaperone must notify staff if the recipient feels or looks at all unwell before leaving as this would necessitate a longer period of observation in the clinic..
